The Revised Personal Questionnaire and Declaration Form: Clarification of Witness Requirement and Special COVID-19 Provisions

Revisions have been made to the Personal Questionnaire and Declaration Form. This update includes a clarification of the witness requirement for its submission. Furthermore, special provisions have been introduced to address circumstances arising from the COVID-19 pandemic.
