Official Information of the Czech National Bank of 10 December 2004 regarding the provision of Section 20a(3) of Act No. 21/1992 Coll., on Banks, as amended by Act No. 257/2004 Coll.

The Czech National Bank, pursuant to Section 20a(3) of Act No. 21/1992 Coll., on Banks, as amended by Act No. 257/2004 Coll. (hereinafter referred to as the "Act"), hereby announces:

1. The bank shall submit to the Czech National Bank an extract of all shareholders and managers from the bank's share issuance.

2. The extract from the bank's share issuance must be prepared on a date that is 7 days prior to the date of the general meeting, and it must be delivered to the Czech National Bank no later than 6 days before the general meeting.

3. Until the central depository takes over the record-keeping of securities maintained by the Central Securities Depository (hereinafter referred to as "CSD"), the bank shall submit a written request to the CSD pursuant to a special law1) for the processing of an extract from the bank's share issuance, which shall contain the data specified in item 1. The bank shall submit the request with justification based on the provision of Section 20a(3) of the Act. The request shall be submitted to the address of the CSD: Rybná 14, Prague 1, Customer Service Section.

4. With the publication of this Official Information in the Bulletin of the Czech National Bank, the Official Information of the Czech National Bank regarding the provision of Section 20a(3) of Act No. 21/1992 Coll., on Banks, as amended by Act No. 126/2002 Coll., published in the Bulletin of the Czech National Bank on 25 June 2003, issue 9/2003, shall cease to be valid.
