The Supervisor under the Alabama Consumer Credit Act requires license holders to submit renewal applications using a prescribed form that includes all mandated information, statements, and representations. This rule standardizes the renewal process by explicitly deferring to the Supervisor's specific formatting and documentation requirements. Promulgated under Code of Alabama 1975, sections 5-19-21 and 5-19-22, the regulation took effect on October 1, 1997.
