INSTRUCTIONS FOR FILLING OUT WEB FORMS FOR MANAGEMENT COMPANIES: DZU UCITS, UAIF, OMD AND DMD

1. Introduction

This instruction contains a description of the content completion for the web forms listed below for management companies: DZU UCITS, UAIF, OMD and DMD. The web forms are publicly available on the Hanfa website (reports.hanfa.hr). Technical details regarding the preparation and processing of web forms, as well as submission to the computer system of the Croatian Financial Services Supervisory Agency (hereinafter: Hanfa), are prescribed by the Technical Instruction for Registration and Operation with the Data Submission Interface in Electronic Form.

The web forms for management companies are:

1. Notifications on Registration and Change of General Data of Supervised Entities Management Companies: DZU UCITS, UAIF, OMD and DMD. By selecting this form type, information is provided on:

   • Change of company name
   • Registration/change of registered office/business address
   • Change of contact details
   • Change of business activities
   • Registration/change of share capital

2. Notifications on Registration and Change of Relevant Business Entities (Legal Persons) Management Companies: DZU UCITS, UAIF, OMD and DMD. By selecting this form type, information is provided on:

   • Registration of founders and registration/change of company members
   • Registration/change of external auditor
   • Status changes – merger
   • Status changes – consolidation
   • Status changes – division

3. Notifications on Registration and Change of Relevant Business Entities (Natural Persons) Management Companies: DZU UCITS, UAIF, OMD and DMD. By selecting this form type, information is provided on:

   • Registration/change of management board members and executive directors
   • Registration/change of supervisory board members
   • Registration of founders and registration/change of company members
   • Registration/change of authorized signatories (prokurists)

4. Management Companies: DZU UCITS, UAIF, OMD and DMD – Submission of Electronic Documents Management Companies: DZU UCITS, UAIF, OMD and DMD. By selecting this form type, documentation not standardized through the above three form types is submitted.

2. Forms for Management Companies: DZU UCITS, UAIF, OMD and DMD

Management companies: DZU UCITS, UAIF, OMD and DMD submit documentation to Hanfa in the manner prescribed by this instruction, in accordance with their obligations arising from the Act on Open-ended Investment Funds with Public Offer (DZU), the Act on Alternative Investment Funds (UAIF), the Act on Mandatory Pension Funds (OMD), the Act on Voluntary Pension Funds (DMD), and subordinate legislation (e.g., primarily regular reports and reports requested by Hanfa according to the respective regulations).

For each web form type, the following fields must be completed as shown in the table and image.
