The form mandates the provision of specific corporate details, statutory documents, activity descriptions, governance structures, and evidence of financial resources including solvency and liquidity. It also requires completed notification and reliability assessment forms for all individuals determining or influencing the institution's policy, along with a description of sound business conduct practices. The completed application and attachments must be sent to DNB's Market Access Expertise Centre, with fees handled separately.
