Guidelines to the Submission of the Statement of Source of Wealth and Source of Funds
The MFSA mandates the use of the Annex AX26 template for the submission of the Statement of Source of Wealth and Source of Funds, replacing the previously required audited Statement of Wealth. This self-declaration form must be completed electronically, signed by the applicant, and submitted as an attachment to the online personal questionnaire at the application stage by all qualifying shareholders and Politically Exposed Persons. The requirement comes into force immediately upon publication, and the information must be updated whenever new material developments arise.

Guidelines to the Submission of the Statement of Source of
Wealth and Source of Funds
Introduction
The Authority is hereby issuing this Circular to inform the industry regarding the publication
of a template (“Annex AX26: Source of wealth and Source of funds self-declaration form”)
to be used for the submission of a statement of source of wealth and source of funds. This
template has been conceived in order to standardise the quantity and quality of the
information provided in this regard. The submission of the statement in this format is
mandatory and will come into force immediately upon publication. For the avoidance of
doubt, the submission of this self-declaration form replaces the previously required audited
Statement of Wealth.
Statement of Source Wealth and Source of Funds: Submission
The Statement of Source of Wealth and Source of Funds refers to the origin of the entire
body of wealth of the individual and to the activity, event, transaction, contract, business,
occupation, or employment from which the funds used are generated or derived.
The statement is to be submitted by all qualifying shareholders and Politically Exposed
Persons (“PEPs”) as attachment to the online personal questionnaire at application stage
and the information should be updated from time to time where new material developments
arise. The statement is to be filled in electronically and signed by the applicant.
These guidelines shall be read in conjunction with the Guidance Note on the Submission of
the source of wealth and source of funds 1
. As indicated in said Guidance Note, the Authority
reserves the right to request the submission of a Source of wealth and Source of funds selfdeclaration form where circumstances so warrant.
