Colin Dowling, Chicago, 125073429 - Temporary medical permit reprimanded due to being terminated from the Cook County Family Medicine Residency Program in July 2023. Bradley Neuman, Chandler, AZ, 036169150 - Physician and surgeon license reprimanded due to discipline by the Arizona Medical Board in October 2025 for failure to properly treat abdominal pain and blockage of a patient. Soumya Pandalai, Phoenix, AZ, 036179491 - Physician and surgeon license issued and placed on indefinite probation for a minimum of five years retroactive to November 8, 2022, with multiple conditions including participation in an approved program due to Applicant's license being disciplined by the State of Arizona in February 2024. Danielle Balding, Olney, 041401498 - Registered professional nurse license shall be indefinitely suspended for a minimum of one year after the Indiana State Board of Nursing indefinitely suspended Respondent's nursing license for a minimum of one year after she failed to perform CPR, instruct certified nursing assistants to perform CPR, and failed to dial 911 after finding a resident unresponsive. Respondent took a drug test which revealed the presence of opiates and benzodiazepines.
