Subject: Amendments on the Authorization Form for Querying the Bangko Sentral Records for Screening Applicants and Confirming Election of Directorsrt'rustees and Appointment of Officers The Monetary Board. in its Resolution No. 4/5 dated 14 May 2026. approved amendments to Appendix 801Q-44 of the Manual of Regulations for Banks (MopB)/Manual of Regulations for Nori-Bank Financial Institutions (MORNBFl) on the OFFICEOFTHE GOVERNOR CIRCULAR No. 1236 Authorization Form for Querying (AFQ) the Bangko Sentral Records for Screening Applicants and Confirming Election of Directorsh'rustees and Appointment of Officers, The amendment requires inclusion of the following details in the AFQ:
Complete Maiden Name of Married Female. Taxpayer Identification Number. Birth Date. Sex. and Last Known Address of the individual subject to the query. This additional information serves as an added layer of verification by the Bangko Sentrat against its existing records to ascertain the individual's qualification or non￾qualification to hold any position in Bangko Sentral Supervised Financial Institutions (BSFls), The amendment also aligns with existing regulations requiring BSFls to use the AFQ￾Attached as Annex A of this Circular is the amended Appendix 801Q-44 of the MORB/MORNBFl. This Circular shall take effect fifteen (15) calendar days following its publication in the Official Gazette or in a newspaper of general circulation.
