With reference to the above subject, and in continuation of our Circular No. (2005/41) dated 03/03/2005, please provide us semi-annually starting from 30/06/2019 with a statement containing the following data:

Employee Full Name	Branch or General/Regional Department where employed	Job Title	Educational Qualification	Appointment Date	Marital Status	ID Number	Place of Residence	Email Address	Mobile Number

The aforementioned statement must be submitted to us in Excel file format within two weeks from the date of the required period.
