SS1/26 – Operational resilience: Incident reporting

Firms must report operational incidents to the Prudential Regulation Authority if they disrupt external service delivery or impact data confidentiality, integrity, availability, or authenticity. Reporting is mandatory when incidents pose risks to UK financial stability, firm safety and soundness, or policyholder protection, assessed via factors such as contagion, reputation, legal obligations, service adequacy, and data safeguarding. Firms must submit reports in three phases using FCA Connect: initial submission as soon as practicable, intermediate updates for significant changes, and final submission within 30 working days of resolution. This supervisory statement applies to UK banks, building societies, PRA-designated investment firms, UK branches of overseas banks, UK Solvency II firms, and the Society of Lloyd’s, becoming effective on 18 March 2027.
