The bulletin mandates that non-high deductible health plans reimburse providers at specific statewide rates defined by the Oregon Health Authority, with no cost-sharing applied to consumers. For high deductible health plans, issuers are not required to cover these services before the deductible is met, as the services do not qualify as preventive care under federal law.
