SEBI mandates that all registered intermediaries, including Depository Participants, Mutual Funds, Portfolio Managers, Collective Investment Schemes, and Venture Capital Funds, adopt a uniform KYC form and supporting documents for investor onboarding. The standardized form, detailed in Annexure-1, captures basic identity and address details in Part I, while intermediaries must collect additional activity-specific information in Part II as prescribed by Depositories, AMFI, or the intermediaries themselves. Compliance with these uniform requirements is required for all new clients from January 1, 2012.
